MEDICAL DOCUMENTS INCLUDE

  • Medical Power of Attorney-allows someone to make healthcare decisions on behalf of another person if they become unable to make decisions themselves.

  • Advance Healthcare Directive-also called a living will, this document states a person’s wishes about medical treatments, life support and end of life care.

  • Do Not Resuscitate (DNR) Orders-some states require notarization for certain versions of DNR directives, which state that medical staff should not perform CPR if the patient’s heart stops.

  • Medical Affidavits-These are sworn statements related to medical matters such as: Disability claims, Insurance claims and Statements about medical conditions.

A valid and current government issued ID with a photo and signature is required for all appointments. Examples include:

  1. U.S. Drivers License

  2. Sate Issued Identification Card

  3. U.S. Millitary Identification

  4. U.S. Passport

  5. Driver’s License or ID from Mexico or Canada

  6. Foreign Passport must include a U.S.immigration stamp or visa documentation (notary services)

  7. Inmate Identification (Issued by Texas Department of Criminal Justice)

If the signer has NO ID (Texas law may allow)

  1. Personal Knowledge (the notary personally knows the signer). or

  2. A credible witness (who knows the signer and has a valid ID).

IDs That Are NOT Acceptable

A Texas notary cannot accept

  1. Expired ID’s

  2. Social Security Cards

  3. Student ID’s

  4. Work badges

  5. Gym membership cards

  6. Medicare/insurance cards

  7. Temporary driver’s license without photo ID

A Notary does NOT

  • Give legal advice

  • Explain legal documents

  • Decide if a document is valid

$250.00

I provide mobile loan signing services, ensuring that real estate and loan documents are accurately and efficiently signed and notarized. I work directly with lenders, title companies and borrowers to ensure a smooth, compliant closing process.

Medical Documents
$10.00

Medical Power of Attorney, Advance Healthcare Directive, HIPPA Authorization Form, Do Not Resuscitate (DNR) Orders, Medical Consent Forms and Medical Affidavits.

*I provide notarization services for certain medical related documents usually to identify the identity of the person signing and confirm the signature is voluntary. I do not certify the medical information-only the signature.

Travel fee of $75.00 applies to this service in addition to notarization fee.

General Notary Public
$10.00

I provide professional and efficient notarization’s for everyday legal and personal documents. Common types of general notary work include Acknowledgements (contracts, property agreements and legal forms) and Jurats (sworn statements) the signer must swear or affirm that the information in the document is true and sign in front of the notary.

*Travel fee of $75.00 applies to this service in addition to notarization fee.
